Triplock karabiners can 'roll' open

While karabiners have been widely used for many years and generally perform well it is believed that they can still be prone to failure for example by the sleeve unintentionally loosening through use, by frictional contact with other equipment or simply by being inadequately tightened over the gate. The inadequacies of the triplock design are currently being discussed worldwide, specifically in relation to dirt and sand ingress making the triplock gate inoperative.
